New Patients

New patient? No problem. We’re happy you’re interested in seeing us. Here are a few things to keep in mind before your visit.

Please bring your:

  • Picture ID
  • Insurance Card
  • and come 15 minutes earlier than your scheduled appointment time to fill out paperwork.

Filling out paperwork and verifying your insurance takes quite a bit of time for you and our front desk team. If you’d like to make the check-in process faster, print out these forms and fill them out in advance.

These forms ask for pieces of information like your name, address, insurance, etc. But the form also asks questions about your health history, including but not limited to chronic medical conditions, allergies, and the health history of your family. The more we understand about your health history, the better equipped our doctors will be in treating you.

Records Release Request

This form gives us your permission to request that your health records be sent to us from a previous doctor you may have seen in the past. It helps us have a better understanding of your health based on these visits.

It also gives us your permission to forward your health records to other physicians who request them. This means that if you need to see a specialist (like a cardiologist), we can send them your health records from your visits with us so that they understand more about you.
